Polite Reminder to Parents/Carers from the School Age Immunisation Team.

We shall be visiting school on Tuesday 3rd November to deliver the Nasal Flu spray programme for Reception - Year 6 children.

Thank you to those who have returned a completed consent form already. If you have not done so and would like your child protected against Flu please complete the consent form as soon as possible.
Please contact the team on 0300 124 0366 to have a form emailed or to complete a verbal consent form over the phone.

On the day of vaccination all children that have received the vaccine will have a smiley face stamp on their wrist to identify that they have had the vaccine, if your child has any allergies please make the team aware by phoning 0300 124 0366.

Thank you.

It is important that parents are made aware of the session date so that they can notify our team if there are any changes in their child’s health conditions particularly asthma or if their child has received the vaccination via their GP practice, since completing the consent form.

Parents of Children in Rec – Yr 6  - Regarding Flu spray it’s not too late to consent. Children with asthma who have increased inhaler use, wheezy 72hrs before or taken oral steroids within 14 days MUST call  0300 124 0366

September 2020

Dear Parent/Guardian

Eligibility for The Childhood Flu Vaccination Programme 2020-2021 will include:  All Schools – Reception to Year 7.

The School Age Immunisation Team is moving to Electronic consents for the Flu vaccination programme.  The use of E consent is quicker and easier to use, and can be completed on any electronic device. In the event that this is not suitable, a paper copy can be obtained from your school.

This vaccination is free and is a quick and simple spray up the nose.  Even if your child had it last year, it is recommended to have the flu vaccine again this year.  If your child has a long-term medical condition they can also access this vaccine at your GP surgery and for those children that cannot have the nasal spray they should have the injected flu vaccine.

The nasal Flu vaccine contains products that come from pigs (porcine gelatine).

Please access more information about Flu and the vaccinations that are available using the link provided at the bottom of the letter:

If there are any changes to your child’s health 14 days before to the vaccination date YOU MUST contact the immunisation team on 0300 124 0366 as your child may not be well enough to receive the vaccination.

  • Your child is asthmatic and becomes wheezy
  • Has required oral steroids
  • Has increased their asthma medication

Vaccinating your child helps to protect against Flu and protects vulnerable friends and family by stopping the spread of flu diseases.

Electronic Consent

Click on this link https://forms.mpft.nhs.uk/  to complete the consent form.  If possible please access the link via Google Chrome.

You will need this code JBFSTOKEMIN which identifies your child’s school

Please complete the e-consent form by 1st November 2020.
